Ask the treating team

Request the discharge summary, written instructions, follow-up plan and information about symptoms that require prompt medical review.

Clarify nursing needs

Identify which tasks are clinical and who is authorised to perform them. Do not ask a non-clinical caregiver to perform nursing procedures.

Prepare the home

Discuss mobility, transport, access, resting space and any equipment recommended by the treating team.

Agree the support plan
