Patient & Family Guide
Helpful points for families considering home-care support.
Start with the person’s preferences
Where possible, include the person receiving care in the conversation. Ask about preferred routines, privacy, language, food preferences, activities and how they would like family members to be involved.
Describe the support needed
Explain which daily activities need assistance, when support is required and what the family can continue to manage. Keep the enquiry focused on general care arrangements rather than sending sensitive records online.
Share relevant instructions safely
For clinical arrangements, make current written instructions from the treating team available through an agreed secure process. Do not ask a caregiver to interpret or change a prescription.
Agree communication
Nominate a family contact, agree how updates will be shared and clarify who should be contacted if the person’s condition changes.
