Family Care Coordination Guide
Suggestions for families coordinating communication and daily support at home.
Choose a main contact
Agree which family member will coordinate non-urgent updates and service questions. Share only necessary contact details with the assigned team.
Keep routines visible
A simple schedule can help family members understand preferred meal times, personal-care routines, visits and agreed handovers. Clinical instructions should remain separate and should come from the treating team.
Clarify who decides
The person receiving care and their lawful representative, where applicable, should be involved in choices. Clinical decisions remain with the treating professionals and the patient as appropriate.
Review when needs change
If daily activities or clinical needs change, discuss the change with JNS and the treating healthcare team before adjusting the assigned tasks.
